Is Confidential Is available to anyone with Medicare Provides group, individual, and family therapy as well as medication management for mental health diagnosis Arranges transportation and a noon meal during the treatment day Coordinates the patient's care with the individuals Primary Care Physician Lasts for 6-12 weeks (at 2-4 days per week) Provides after care program following discharge

This outpatient program assists Adults and Senior Adults
struggling with: Depressed Mood Unresolved Grief Anxiety, nervousness Worry Isolation/Loneliness Difficulty coping with physical changes Sleep or appetite changes Feeling of hopelessness or helplessness Anger Low self-esteem Personality changes
